Medical Nutrition Therapy

Personal Consultation

$135.00

This is a one-on-one session with a registered dietitian and consists of an intitial 60 minute session and a 30-minute follow up session. The 60-minute intitial consultation includes:

An in-depth nutritional assessment (print form from the link below)
Body measurments (% body fat, weight, hips, waist and arm)
Goal setting
Meal plan
3 day food record anaylsis

The 30-minute follow-up appointment is to follow up on your progress and address any questions or concerns.

Nutrition Assessment Form

Inividual Follow-Up Session	$50.00
This is for those individuals that have already had an initial consultation and need more than one consultation you can spend 30-minutes with the registered dietitian to answer any additional questions that you may have and track your goals.
Follow-Up Maintenance	$210.00
Includes 5-30 minute visits with the registered dietitian.

Payment is due at the time of service. Amounts do not include tax.

Medical insurance companies may or may not offer coverage for medical nutrition therapy or preventative care. Carefully investigate the type of coverage you have. It is your responsibility to pay for your visit and to have your insurance company reimburse you if applicable. You will be provided with a Super bill that you can submit to your insurance company for reimbursement. You will be responsible to submit claims directly to your carrier.
